Even with an excellent insurance policy, it does not mean you have coverage for nutrition services. To verify your nutrition coverage, please call the 800 number on the back of your insurance card and ask to speak with a representative from member services. Use my name as the provider. If asked, you can provide them with my NPI # 1215038666.
Please ask the representative the following questions:

- Are nutrition services with a registered dietitian nutritionist including education and counseling a covered service on my current policy?
- Is my diagnosis covered? (You need to get a diagnosis code from your doctor). Be sure to ask the representative to check both medical and preventative coverage.
- How many covered visits do I have per calendar year?
- What is considered the calendar year for my current policy?
- Do I have a deductible I need to meet for nutrition services before the visits are covered by my insurance?
- Do I have a co-pay or co-insurance for nutrition services?
- May I please have a reference number for this call?
You will be required to pay any applicable co-pays (co-insurance) at the time of the visit. You will also sign a liability waiver stating that you will be responsible for the charges if your insurance does not cover the visit.
